Select Specialty RX provides licensed pharmaceutical services including, but not limited to:

• Prescription fulfillment and specialty medication dispensing.
• Prescription transfer coordination from other pharmacies.
• Immunizations including flu shots, COVID-19 boosters, shingles, and pneumonia vaccines.
• Prior authorization assistance and insurance coordination.
• Medication therapy management and pharmacist consultations.
• Free medication delivery throughout Southern California.
• Refill synchronization and medication management support.

All pharmaceutical services are provided in accordance with California state pharmacy law and applicable federal regulations.

All prescription medications require a valid prescription from a licensed healthcare provider. By submitting a transfer request or prescription to us, you confirm that:

• The prescription is valid, current, and issued by a licensed prescriber.
• The medication is intended solely for your personal use or for the patient named on the prescription.
• All information provided is accurate and complete to the best of your knowledge.
• You authorize us to contact your prescriber or current pharmacy to facilitate your care.

We accept all Medi-Cal plans and work with most major insurance providers. By providing your insurance information, you authorize us to:

• Submit claims to your insurance on your behalf.
• Verify your eligibility and benefits prior to dispensing.
• Process prior authorizations as needed.

You are responsible for any copayments, deductibles, or non-covered amounts as determined by your insurance plan. We will inform you of any out-of-pocket costs before dispensing your medication.

We offer free medication delivery to all locations within Southern California. By requesting delivery, you agree to the following:

• Provide a valid, accurate delivery address within our service area.
• Ensure someone is available to receive controlled substances or medications requiring a signature.
• Notify us of any changes to your delivery address in advance.
• Delivery times are estimates and may vary based on location, traffic, and prescription readiness.

Select Specialty RX is not responsible for delivery failures caused by incorrect address information or unavailability of the recipient.

Immunizations are administered by licensed pharmacists or certified pharmacy staff. By receiving a vaccine at our pharmacy:

• You confirm that you have disclosed all relevant medical history and allergies.
• You understand that vaccines, like all medical procedures, carry minimal inherent risks.
• You consent to the administration of the requested vaccine.
• You agree to remain at the pharmacy for the recommended observation period following administration.

Select Specialty RX provides services in good faith and in accordance with professional pharmacy standards. To the maximum extent permitted by law, we are not liable for:

• Adverse reactions arising from patient-provided inaccurate medical history or undisclosed allergies.
• Delays in service caused by insurance processing, prior authorization requirements, or prescriber response times.
• Third-party actions, including insurance denials, prescriber errors, or delivery partner issues.
• Any indirect, incidental, or consequential damages arising from the use of our services.

Nothing in these terms limits our liability for gross negligence, willful misconduct, or any liability that cannot be excluded under applicable law.
